RCAs: Root Causes Identified, Impact Still Unaddressed

1. "Blaming" scenario

It is still seen that teams prefer to pass the ball to other teams instead of showing accountability to fix the issue. Showing trust, transparency and integrity play a crucial role while sharing the data to come with a logical root cause, corrective actions and preventive actions as well.

2. Lack of collaboration

In most of the cases, teams still use the conventional method of routing L1->L2->L3 instead of swarming method. Due to conventional method, there could be bright chances of having communication gap, which leads to non-collaborative culture.

This is why, though the root cause is identified, corrective & preventive actions keep lingering for few months due to dependency.

3. Pre-decided conclusions

During triaging phase, teams come up with root causes. However, they also come up with pre-decided conclusions along with it. Then, RCA becomes the justification activity. Due to which, symptoms are getting fixed and not the root causes.

4. Lack of details:

In few cases, detailed info is missing in documenting the triaging part and hence the info mentioned therein does not carry value and does not show proper direction either.

5. Pressure of meeting Service Levels:

Due to time pressure, cross-functional teams come together for process-sake for discussion. However, the root cause, corrective & preventive actions may be identified in shorter span to meet the SLA without proper deadline to implement and hence action tracker remains in yellow & the problem keeps recurring.

Root Cause Analysis often fails due to blame-driven behavior, weak collaboration, and rushed decision-making.
Sequential escalations and pre-decided conclusions limit transparency and lead to fixing symptoms instead of true causes.

Poor triage documentation further weakens analysis, while SLA pressure forces premature closure without sustainable actions.
As a result, corrective and preventive actions linger, and the same problems recur.
